Facility Overdose Response Box
The FORB program provides overdose response boxes with the opioid antidote naloxone for employees at community-based organizations.
About the Program

The response boxes include 3, 5, 10 or 20 doses of naloxone and other OD response supplies which are supplied at no charge by the British Columbia Centre for Disease Control to sites registered in the program.

Program Eligibility
To be eligible to join the FORB program, your site must:
- work with clients at risk of opioid overdose
- be a not-for-profit organization
Step 1: Review the Overdose Planning and Training Resources
Before you can receive your supplies, it is important that your staff are trained and that your site has reviewed the Overdose Planning Resources.
Before submitting your application, review the Resources for New Sites to ensure the program is right for your organization.

Organization Registration Form
- All information on the registration form is required.
- Designate someone from your head office to communicate with BCCDC about site eligibility and program changes.
- Your executive director will need to sign this form to indicate they are aware that the organization will be participating in the Facility Overdose Response Box Program.

Site Registration Form
- All information on the registration form is required.
- Designate someone from the site to communicate with BCCDC about day-to-day operations and program documentation.
- Only complete 1 form for every address, even if you operate multiple programs from one location.
- Indicate what services are offered at your site, and provide estimates for the number of employees at the site and the number of clients served daily.
- Important: Give the estimated number of employees that will need training in Opioid Overdose Recognition and Response so that training supplies can be sent to you.
- Outreach kits will be provided for sites that offer outreach services – do not submit separate applications for outreach teams.

Complete one form for each site within your organization that needs a Facility Overdose Response Box (even if there is only 1 site and it is the same as the organization).

Step 3: Approval & Next Steps
Confirming your Eligibility
Once your application has been assessed, your organisation will receive a confirmation email from the BCCDC confirming your eligibility and will determine whether your site requires training supplies.
Please refer to the Registered Sites section of the website to review training materials, implementation guidelines and other resources.
Step 4: Submit Final Agreements & First Order
Once your site has received training and reviewed the overdose planning resources, submit the Training Agreement and New Site Agreement to complete the application.
The first time you order F.O.R.B supplies, submit the Initial Supply Order form.

If you choose to submit your application after training and planning for your site is complete, you may use this package which combines all paperwork required to complete your application.
